• Latest BIR Income Tax Return (ITR) of parents
• Certificate of Good Moral Character from the High School Principal or Guidance Counselor
• National Career Assessment Examination Rating
• Certification from the High School Principal that the applicant belongs to the top (10) ten of the graduating class

HALF MERIT

• Form 138 (photocopy signed by the Principal)
• Latest BIR Income Tax Return (ITR) of parents
• Certificate of Good Moral Character from the High School Principal or Guidance Counselor
• National Career Assessment Examination Rating

TULONG-DUNONG:
- SP-IEP(Tribe)

• Form 138 (photocopy signed by the Principal)
• Latest BIR Income Tax Return (ITR) of parents
• Certificate of Good Moral Character from the High School Principal or Guidance Counselor
• National Career Assessment Examination Rating
• Certification of membership either by the Office of Muslim Affairs or the National Commission on Indigenous People

- Solo Parent

• Form 138 (photocopy signed by the Principal)
• Latest BIR Income Tax Return (ITR) of parents
• Certificate of Good Moral Character from the High School Principal or Guidance Counselor
• National Career Assessment Examination Rating
• Affidavit duly executed by the applicant that he/she is a solo parent or a child of a solo parent with an attached certification from a DSWD worker in the area
• Certification of good moral character from the Barangay Chairman of the place of domicile
• Certification of good health
• Birth Certificate
• Notice of admission from the school where the applicant intends to enroll
• ID of solo parent

- SSGP

• Certification from the congressman that the applicant is permanent resident of his/her congressional districts

- SNPLP

• Official enrollment certificate and Statement of Accounts from school enrolled in
• Official report of rating or certificate of grades for the last semester attended issued by the school registrar
• Certificate of guarantee of loan by a GSIS or SSS member
